You can pick up a FUNDRAISER REQUEST FORM outside the SEB office door in the black mailbox, located in the Student Lounge, SSU 202.
Before filling out this form, please make sure to have all arrangements regarding food services through the Dining Services Office and scheduling of rooms through the Office of Conference & Event Services confirmed.
The information that must be completed on the form include:
Before submitting, please make sure to include the following:
Signature of Advisor
Signature of Club President
Student Contact Name and E-mail
**Please submit this form to the Fundraiser box outside the SEB office by Sunday at 7:00pm, one week prior to the start of the fundraiser.
**Also, you MUST attend the SEB meeting at 5:25 located in SSU 241 the Monday after you submit this form to verbally make your request to the board.
** Should you fail to submit a request to fundraise before the fundraising event begins, your club’s account will be FROZEN until further notice.
