All events are subject to College policy, including but not limited to: Siena College Guidelines for Scheduling and Facilities Usage, Speakers' Policy, Alcohol policy.In order to adequately prepare for an event an event complete this form and submit it to Campus Scheduling (at least 5 business days prior to the event). For events that have any items checked in the risk assessment below, this form must be submitted no less than 2 weeks in advance of the planned event.
