Anthem offers reliable, affordable health insurance coverage that can be trusted and is the carrier of choice for local colleges. Anthem is a local insurer and is accepted by most local providers as well as many providers nationally. The Siena student health insurance PPO plan with Anthem, Group # L11030 provides health insurance coverage for inpatient care, outpatient services, prescription medicine, $0.00 cost mental health care, as well as catastrophic illness and injury.
ANTHEM PLAN FAQ'S
WHEN IS THE PLAN EFFECTIVE?
2024-2025 - Anthem coverage is effective August 15, 2024 through August 14, 2025 for the 2024-2025 academic year. For Fall semester only full-time students, coverage is effective August 15, 2024 – February 14, 2025. Coverage for Spring semester only full-time students is effective January 1, 2025 – August 14, 2025.
WHAT DOES THE PLAN COST?
2024-2025 - The Anthem full year plan cost will be $2,333.00 and will be billed in two installments of $1,166.50 (one each on the first tuition bill of each semester.) Spring semester only full-time students will be billed $1,444.55 for coverage from January 1-August 14, 2025. Students who enroll midyear will be charged pro-rata based upon the date of activation. Students who waive or request termination after the waiver deadline will be refunded pro-rata based upon the date of the approved request.

IS BEHAVIORAL/MENTAL HEALTH COVERED BY THE PLAN?
Yes! Anthem will fully cover the cost of in network Mental Health/Substance Use Outpatient Services (virtual AND office) with a $0 copayment. Out-of-network providers are the deductible ($4,000) then 40% Coinsurance.
Inpatient services are the deductible ($500) then 30% Coinsurance for in network providers. Out-of-network providers are the deductible ($4,000) then 40% Coinsurance.

HOW DO I RECEIVE MY ANTHEM INSURANCE MEMBERSHIP CARD?
Plan participants can download and/or print a copy of their insurance card by logging into the Anthem Portal.
Misplaced your ID Card? No problem! Login to your online account to view and print your ID card.

WAIVED THE SIENA HEALTH INSURANCE PLAN (CDPHP) BUT MY OTHER INSURANCE COMPANY DENIED A CLAIM SAYING I WAS ENROLLED IN THE SIENA CDPHP PLAN. HOW CAN I FIX THIS?
Some health insurance plans have a Coordination of Benefits (COB) clause making any other insurance you may have your primary insurance plan. If your insurance company is one of those (Fidelis, etc.), they will deny claims.
